Telehealth Informed Consent

This Telehealth Informed Consent ("Consent") describes the telehealth services offered through Pallas Health, a service of Brentmoor, Inc. ("Pallas," "we," "us"), and the rights and risks associated with receiving care via telehealth. Please read it carefully. Submitting your intake and continuing through checkout indicates that you have read, understood, and agreed to this Consent.

1. What telehealth is

Telehealth is the delivery of healthcare services using interactive audio, video, or other electronic media (including asynchronous review of intake forms, photographs, and messages) between a patient and a clinician who are not in the same physical location. Pallas Health is a technology platform. Pallas does not employ medical providers, does not practice medicine, and does not direct clinical decisions. Clinical care is provided by independent US-licensed clinicians contracted through our clinical partner network — a contracted telehealth provider organization and its affiliated state-licensed professional corporations.

2. Services you may receive

Through the Pallas platform you may receive: (a) review of your health questionnaire, weight history, current medications, medical history, and treatment goals by a clinician licensed in your state; (b) where required by your state's law, a synchronous audio or audio-video visit; (c) clinical evaluation for the appropriateness of GLP-1 receptor agonist therapy for chronic weight management; (d) issuance of an electronic prescription to a partnered US-licensed pharmacy when treatment is clinically indicated and consistent with applicable law; (e) ongoing messaging with your care team for follow-up, side effects, and refills.

3. The clinician–patient relationship

A clinician–patient relationship is established only when a clinician licensed in your state reviews your intake (and, where required, conducts a synchronous visit), determines that telehealth is clinically appropriate for your situation, and accepts you as a patient. The treating clinician retains full independent professional judgment. The clinician may decline to prescribe, may recommend in-person evaluation, or may discontinue care at any time when, in their judgment, doing so is clinically appropriate.

4. Benefits of telehealth

  • Improved access to clinicians without travel.
  • More flexible scheduling and the ability to receive care from home.
  • Continuity of care through asynchronous messaging with your care team.
  • Discreet evaluation and treatment for chronic weight management.

5. Risks and limitations of telehealth

Telehealth may not be appropriate for every clinical situation. You should be aware of the following risks and limitations:

  • Without an in-person physical examination, a clinician may not identify findings that would otherwise be apparent. A telehealth visit is not equivalent to an in-person visit.
  • The information you provide may be incomplete or inaccurate, which can affect the clinician's evaluation. You agree to provide truthful, current, and complete information about your health.
  • Technical failures (poor connectivity, hardware or software problems, outages, or delays) may interrupt or prevent care.
  • Despite reasonable safeguards, electronic transmissions could in rare cases be intercepted or accessed without authorization.
  • Rare adverse outcomes — including unexpected reactions to prescribed medication or progression of an underlying condition that requires in-person evaluation — can occur.
  • Your clinician may determine that telehealth is not appropriate and that you should be evaluated in person.

6. Compounded GLP-1 medications — important disclosure

If your clinician determines that a compounded GLP-1 medication is appropriate for you and prescribes one, you should understand the following:

  • Compounded medications are not FDA-approved, are not generic versions of brand-name drugs, and have not been evaluated by the FDA for safety, efficacy, or quality.
  • Compounded preparations are prepared by a US-licensed 503A compounding pharmacy or 503B outsourcing facility pursuant to a patient-specific clinical-difference rationale documented by your treating clinician.
  • Clinical trial results referenced anywhere on the Pallas website pertain to FDA-approved products (Wegovy®, Zepbound®, Ozempic®, Mounjaro®) and have not been clinically established for compounded preparations.
  • You will receive medication-specific safety information, including risks, contraindications, and side effects, before any prescription is issued.

7. Emergencies and urgent care

Telehealth is not for emergencies. If you are experiencing a medical emergency, severe allergic reaction, chest pain, signs of a stroke, suicidal thoughts, or any other urgent or life-threatening condition, call 911 immediately or go to your nearest emergency department. Pallas does not provide emergency or urgent care services.

8. Privacy and confidentiality

All Protected Health Information (PHI) communicated through the Pallas platform is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Patient medical records are created and maintained by our contracted clinical partner network (the HIPAA Covered Entity providing medical services); Pallas Health (Brentmoor, Inc.) operates the technology platform and acts as a HIPAA Business Associate. 9. No recording without consent

Telehealth visits are not recorded by Pallas or by clinicians in our partner network without your prior, explicit consent. Clinical documentation (chart notes, prescriptions, lab orders, and messages) is created and retained as part of your medical record consistent with applicable state and federal record-keeping requirements.

10. Prescribing limitations

Pallas's clinical scope is limited to GLP-1 receptor agonists for chronic weight management. Clinicians in our partner network do not prescribe controlled substances through the Pallas platform. Your clinician will use independent professional judgment to determine whether a prescription is clinically appropriate; not every patient is a candidate for GLP-1 therapy.

11. Right to refuse or withdraw

Your participation in telehealth services through Pallas is voluntary. You have the right to refuse telehealth services and instead seek in-person care, and you have the right to withdraw this consent at any time. Withdrawal of consent will not affect care already provided but will end the telehealth relationship going forward. To withdraw consent, contact support@pallashealth.co.

12. Alternatives to telehealth

In-person evaluation by a primary care clinician, an endocrinologist, or a weight-management specialist is an alternative to telehealth. If at any point you prefer in-person care, you may discontinue telehealth and seek evaluation locally. Your clinician can also recommend in-person care if they determine it is more appropriate for your situation.

13. Technology requirements and failures

You are responsible for the device, internet connection, and electronic communications you use to access the service. If a technical failure interrupts a synchronous visit, your clinician will reach out to you using the contact information on file to reschedule or complete the encounter by an alternate method.

14. State law and licensure

Telehealth services are provided only to patients located in US states where our partner clinical network is licensed and where applicable state law permits the services we offer. The current list of states served is published at /clinical. You agree to truthfully report your physical location at the time of each encounter; care cannot be provided to patients located outside our serviced jurisdictions.

15. Acknowledgment and consent

By checking the telehealth-consent box during intake and continuing to checkout, you acknowledge that:

  • You have read and understood this Consent.
  • You have had an opportunity to ask questions about telehealth and the services offered through Pallas, and your questions have been answered to your satisfaction (or you have chosen to proceed without further questions).
  • You understand the benefits, risks, and limitations of telehealth as described above.
  • You consent to receive telehealth services from independent US-licensed clinicians contracted through our partner clinical network, on the terms set out in this Consent, our Terms of Service, and our Privacy Policy.
  • You are at least 18 years of age and located in a US state served by Pallas.
