The Wegovy® pen is Novo Nordisk's once-weekly, single-dose injection pen for semaglutide, FDA-approved for chronic weight management. It's one of the simplest injection devices you can be prescribed: each pen contains exactly one preset dose, the needle stays hidden the whole time, and there's nothing to dial, prime, or attach. Before you start
You'll need: your Wegovy® pen, an alcohol swab, and a sharps disposal container. That's it — the needle is built into the pen and never needs attaching, and because each pen is single-use with a preset dose, there is no dial to turn and no priming step.
Wegovy® comes in five dose strengths (0.25 mg, 0.5 mg, 1 mg, 1.7 mg, and 2.4 mg), and the pens you receive match your current step on the schedule your clinician set. Check the label before every injection — during dose escalation it's easy to have two strengths in the fridge at once.
Step by step
- Wash your hands. Wash thoroughly with soap and water before handling the pen.
- Check the pen and medicine. Confirm the label shows Wegovy and the dose strength your clinician prescribed, and that it isn't past the expiration date. Look at the liquid through the pen window — it should be clear and colorless. Don't use a pen that looks used or damaged, has been dropped or frozen, or contains cloudy or discolored liquid.
- Let it reach room temperature. A pen straight from the fridge can sting. Letting it sit out with the cap on for the time noted in your Instructions for Use makes the injection more comfortable.
- Choose and clean a site. Inject into the abdomen (at least two inches from the belly button), the front of the thigh, or the back of the upper arm. Move to a different spot than last week, wipe the site with an alcohol swab, and let the skin dry.
- Remove the cap. Pull the cap straight off only when you're ready to inject. The needle stays hidden inside the needle cover — don't touch the cover or press it against anything before the injection.
- Press the pen firmly against your skin. Hold the pen straight against the site at a 90-degree angle and press down firmly. The injection starts automatically — you'll hear a click and see the yellow bar in the window begin to move.
- Hold until the dose finishes. Keep pressing the pen against your skin until the yellow bar stops moving, then continue holding for the full count in your Instructions for Use. Lifting the pen early can mean an incomplete dose.
- Dispose of the whole pen. Lift the pen straight off and place the entire used pen in an FDA-cleared sharps disposal container. Each pen is single-use — never save or reuse one.
Where to inject (and rotating sites)
The three approved injection areas are the abdomen (at least 2 inches from your belly button), the front of the thigh (either thigh), and the back of the upper arm (someone else may need to help you inject there). You can use the same general area each week, but move to a different spot within it — injecting the exact same spot repeatedly can cause lumps or irritation that affect how the medication absorbs.

The dose schedule, briefly
Wegovy® is titrated: treatment starts at the lowest strength and steps up gradually — typically every four weeks — until you reach the maintenance dose your clinician targets. The slow ramp exists to let your body adjust and to keep side effects manageable, so don't skip steps or move up on your own. Your prescriber sets the pace, and it's normal for them to hold a step longer if you're having side effects.
Storing Wegovy pens
- Before use: keep pens refrigerated at 36–46°F, in the original carton to protect them from light. Never freeze a pen — if one freezes, throw it away even after it thaws.
- Room temperature: if needed, an unused pen can be kept at 46–86°F for up to 28 days in its carton. After 28 days out of the fridge, discard it — returning it to the refrigerator doesn't reset the clock.
- Always: keep the cap on until you're ready to inject, and store pens out of reach of children and pets.

- Removing the cap early. Take the cap off only when you're ready to inject — the needle cover shouldn't be touched or pressed against anything before the shot.
- Not pressing firmly enough. The pen only starts injecting when it's pushed firmly against your skin. Weak contact means no injection, or a partial one.
- Pulling away too soon. Lifting the pen before the yellow bar stops moving can mean an incomplete dose. Hold it down for the full count in your Instructions for Use.
- Injecting cold medication straight from the fridge. Letting the pen warm up first (cap on) is more comfortable and can reduce stinging.
- Skipping site rotation. Same-spot injections lead to lumps and uneven absorption.
- Reusing or saving a pen. Each pen is one dose. Once used, it goes in the sharps container — even if it looks like liquid remains.
